Forms Needed for Bulk Mail Distribution of the Newsletter

The monthly "bulk" mailing of the Palm Beach County Mensa newsletter must be accompanied by two forms. Completed samples of these forms are included in the images below. (The small "thumbnail" versions below are linked to large versions of the forms.)

The first of the two forms required is issued by the postal service. It is form PS Form 3602-R, January 2001. The front part of this form is shown in the following image.

The back side of PS Form 3602-R is shown next.

The CASS certification form provided by the national office is shown below.
